1. Contact information: The company (or technician) and the client’s name, address, phone number, and email.
2. Work request: What the issue is, the type of appliance, and where it is located.
3. Cost estimate: List of every part needed, the cost of each, serial numbers, and the estimated labor cost for conducting the repair(s).
4. Approval: Before work should begin, the customer needs to view the order and sign their approval.
